You can also write to: Office of Academic and Student Affairs UMDNJ-School of Health Related Professions 65 Bergen Street Newark, New Jersey 07107-3001 Application Procedures: The following documents must be submitted for application: | 1- | Copy of Current nursing license. | | 2- | Copy of all recent continuing education documentation (including, BLS, Neonatal resuscitation, Fetal Monitoring Workshops). | | 3- | Official transcripts of all the schools attended, to be sent directly to the office of Academic and Student Affairs (see address above). | | 4- | Three reference letters, from professionals who have worked with you during the past five years, including your last or present employer. | | 5- | Transcript from recent Physical Assessment Course. | | 6- | A short essay ( about one type-written page) describing yourself and expressing your goals. | All applications must be received by February 1st. All credentials must be in your file by February 15th or your application cannot be forwarded to the Admission committee. A personal interview, scheduled in March with the program's faculty, will be required.
