is seeking a qualified candidate to fill the following full-time
position:
Supportive Services Worker
Join our service team providing supportive housing services
to our tenants diagnosed with severe and persistent mental
illness. Help support them be successful in their independent
living situations. Assist program residents' re-establish
valued social roles in the community. Work in
a variety of ways to promote self-sufficiency.
Flexible hours. Excellent benefits package. Related experience
and/or CPRP an asset. Computer experience is a major plus.

Mental
Health Advocate- PACT III - We are seeking individuals
who have experience in family support counseling, including
assessment and interventions in conjunction with consumer.
B.A. with a min. of 2 years experience in the mental health

Family
Support Case Manager PACT IV - Assesses and develops
plans and interventions in conjunction with the consumer,
family and the team in the area of family support. B.A. with

Rehabilitation Specialist- PACT I- Assesses
consumers' education and employment history and develops plans
and interventions in conjunction with the consumer and the
team in the area of employment/career services.
MENTAL
HEALTH ADVOCATE/ PEER ADVOCATE- PACT I - As part
of a multi-disciplinary team, provides assessment and direct
service to seriously and persistently mentally ill consumers
who are enrolled in the PACT Program. Responsibilities include:
Case management, counseling, crisis intervention and mental
health education.

providing direct services to individuals with developmental
disabilities and/or serious mental illness.
Requirements
Bachelor's
degree in human services or related field preferred; experience

Responsibilities
Provide
direct social services to individuals with developmental disabilities
and/or mental illness; including personal visits, coordination
of services, and advocacy for additional and improved services

program taff and other service providers as part of an interdisciplinary
team.
Communicate
with family members as needed regarding status and progress
of their family member and provide or assist enrolled
families with emergency response, crisis management and negotiating
the appropriate service system as needed.
